The most common reason for people 65 years of age or older being conserved is Dementia of which alzheimer’s is the most notable form/type of dementia, although there are over 70 different forms/types of dementia.
Our objective is to educate the senior participants and underserved communities at-large about conservatorships and the many functions therein (Probate court, rights they have, freedoms lost, attorney’s etc.). We also build confidence and empower participants by encouraging a proactive stance, taking charge of their affairs, which helps reduce stress and create peace of mind.
We have created a document which, after completing and notarizing, will become an important legal document once notarized and component of the participants estate plan. The document is called “Advance Personal Care Directive”. Participants write down the things they like, things that make them happy, places they like to go, music they love, favorite foods, etc., also things they do not like, things that makes them sad, places they feel uncomfortable going, people they do not like visiting or being around, etc. It is a “New Way” of memorializing individual needs and making personal preferences known in the case of incapacity. We believe this exercise can also aid participants to take a closer assessment of their overall lifestyle, both, at the present and in the future.
